About Privileges

Process for Medical and Allied Health Professional Staff Privilege Application

  • Contact Lori Salmons at the Medical Staff Office at the Hospital at (805) 653-6434 x 208
  • If leaving a message, please include your Name, Mailing Address, License (MD, PhD, MFT, LCSW) and a phone number where you can be reached.
  • We will send you an application for membership and privileges to complete and return.
  • Once the completed application and $150 application fee are received, the Medical Staff Office will process the application and obtain all verifications and references.
  • When all the required information is received, the completed tile will be reviewed by the hospital's medical executive committee and the governing board.
  • Following the governing board's approval, a letter will be forwarded to you from the medical staff office indicating your staff status and privileges that were approved.
  • You will be assigned a proctor from a peer on the staff to review at least your first 3 cases.

Temporary Privileges

  • Temporary priviliges can be requested from the Medical Staff Office for your own patient that has been admitted to the hospital.

  • Fax over a copy of your license, DEA (if applicable) and Malpractice Certificate to (805) 653-6248
  • The Medical Staff Coordiator will as you a few questions to complete the Temporary Privilege Application and get approvals from the Medical Director and CEO.
  • Temporary priveleges for individual patients can be requested twice a year. If your patient activity will be more than that, you should apply to the Medical and Allied Health Professional Staff as described above.
